Information for Patients
  • Appointments Office visits are by appointment only. We urge you to call as far in advance as possible for your routine appointments. If you must cancel an appointment, please give us 24 hour notice so that we may offer your appointment time to another patient. Failing to cancel an appointment will result in a "no show" fee in the amount of $50.00. We ask that our patients do their best to be on time for their appointments. Lateness is unfair to the other patients who are being seen that day. Occasionally, our providers will have emergencies that will delay them. We ask for your patience and understanding.
  • Urgent Visits Each day we have a limited number of same-day urgent visit slots reserved. Please inform the receptionist if your matter is urgent; we will do our best to accomodate your needs.
  • After Hour Emergencies During the evenings, weekends and holidays, one of our providers is always on call and available to handle emergencies for established patients. Please leave a clear message with a return phone number. Inactivate the caller-ID block on your phone to allow the provider to return the call.
  • Annual Exams We would like you to arrange for routine blood testing (fasting) about 5 days before your appointment for your annual exam. These can be performed at our office or at a drawing station of your choosing.
  • Medication Refills It is always best to obtain any refills at the time of your visit. Due to insurance restrictions and risk management concerns, we now send prescription refills to pharmacies electronically. When calling for a prescription kindly have the following information available: spelling of medication, strength and dosage, pharmacy information, and your phone number. Please allow 1 business day to complete your prescription request.
  • Referral Requests If the request is for a new referral, an appointment is needed with your provider. Otherwise, please contact the front desk with your referral request.
  • Forms Please make an appointment with your provider to complete employment and/or school forms.
  • Office Visits and Insurance Please have your insurance ID card with you for each visit. If you have any change of insurance plan, home address, phone number or employment, let us know. Please be prepared to pay any visit fees including copayments at the time of your visit. We accept credit card payments as well as cash and checks.
  • Specialist Care and Hospitalization We have established relationships with a select group of specialists who works closely with us. They have been prescreened to ensure optimal care and a close working relationship. If you need hospitalization, we will coordinate your hospital care with the hospitalist who will manage your care during your hospitalization. The "hospitalist" is a physician who specializes in in-patient care. We have an exclusive relationship with the hospitalist and work closely coordinating your care during your entire stay.
  • Billing and Patient Account We have a billing/patient account team to assist you with any insurance payment concerns and issues including yearly deductibles. We want to remind you that it is your responsibility to check with your insurance company regarding your copayments, yearly deductibles, and covered services (e.g., vaccines) before your scheduled appointment to avoid any potential financial problems.
