Office Policies

Welcome to Astor Medical. Our practice's main goals are quality patient care and patient satisfaction. We appreciate your selection of this office to serve your health needs. We hope the following information is useful for you.

Office Hours - Monday-Thursday 10AM-6PM, Friday 9AM-4PM

Appointments - Office visits are by appointment only. We urge you to call as far in advance as possible for your routine appointments. If you must cancel an appointment, please give us 24 hours notice so that we may offer your appointment time to another patient. Failing to cancel an appointment will result in a "no show" fee in the amount of $50.00. We ask that our patients do their best to be on time for their appointments. Lateness is unfair to the other patients who are being seen that day. Occasionally, our providers will have emergencies that will delay them. We ask for your patience and understanding.

Urgent Visits - Every provider has daily urgent visit slots. If your matter is urgent, please inform the receptionist when you call; we will do our best to accommodate your needs.

After Hours Emergencies - During the evenings, weekends, and holidays, one of our providers is always on call and is available to handle emergencies for established patients. Please leave a clear message with a return phone number and inactivate the caller-ID block on your phone to allow the provider to return your call.

Annual Physical Exams - We would like you to arrange for routine testing (fasting) about 5 days before your appointment for your annual exam. These can be performed at the office or a drawing station of your choice.

Medication Refills - It is always best to obtain any refills at the time of your visit. Due to insurance restrictions, we are unable to phone prescription refills to pharmacies. Please call our office during business hours to make refill requests.

Referral Request - If the request is for a new referral, please make an appointment with your provider.

Forms - Employment and school forms generally take 4-5 business days to proceed. There is a $35 forms fee.

Test Results and Routine Matters - If your provider cannot speak with you at the time of our call, our staff will take your number and have the provider call you back within one business day for all routine matters. If the matter is urgent, please inform the receptionist. You may also use the Patient Portal for non-urgent matters.

Insurance - The insurance contract is ultimately between you and your insurance company. It is important for you to provide our staff with correct and complete insurance information. Please check to see that your provider is identified as your primary care physician and familiarize yourself with your plan's copays, immunization policies, and referral procedures.
